-Alumni Transcript & Certificate Requests-

Upon graduation, students free and clear of outstanding obligations receive their school certificate, unofficial academic transcript and American Heart Association CPR/AED Certification.  

The Registrar can produce additional academic transcripts and certificates of completion at the request of alumni.  There is a five (5) dollar processing fee for each official transcript or duplicate certificate requested.  Money order and credit card are accepted however personal checks are not. To request an official transcript or duplicate certificate simply download the request form below, complete it, providing all information required.  Mail the completed form along with required fee (s) to the school Registrar.  The request is generally honored within 7 business days of its receipt.

Student Verification Letter Requests

At the student’s request, the Registrar’s office can provide students currently attending The New York School for Medical or Dental Assistants with a letter verifying their student status. Verification Letters are generally available for pick-up at the school’s Main Desk within 24 hours of submitting the request. Submit the completed request in person to the Registrar’s Office or email it to Registrar. Pre-printed Verification Letter Request forms are also available at the Main Desk and in the Registrar’s Office.

Change of Address/Contact Info

If you are a student or graduate who has had a change in contact information (telephone numbers, email or mailing address) since first enrolling in the school, please complete and submit this Change of Address / Contact Information Form (pdf) to keep your school records current.
